About this exercise

The weighted bottle side-lying shoulder external rotation is a rotator cuff strengthening exercise performed lying on one side with a water bottle or similarly light improvised weight. By rotating the forearm upward from a bent-elbow position, it isolates the infraspinatus and teres minor — key muscles for shoulder health and stability.

How to Perform

  • Lie on your side with your bottom arm supporting your head or extended on the floor
  • Bend the top elbow to 90 degrees so the forearm points across your torso, holding a filled water bottle or light weight in that hand
  • Keeping the elbow pinned against your side, rotate your forearm upward until it points toward the ceiling
  • Pause briefly at the top without letting the shoulder shrug
  • Slowly lower the forearm back to the starting position
  • Complete all reps on one side before switching
